GDPR Article 15 Subject Access Request — Medical / Healthcare File

A formal request for every record the GP / paediatrician / hospital holds on your child. Same free / 30-day / no-question-asked basis as the school SAR. Available to any parent with parental responsibility.

Particularly valuable for revealing: missed appointments (with who-brought-the-child entries), the alienating parent's account of medical concerns (often different from what they later tell the court), and any clinician's notes recording the child's expressed wishes about contact.

I am writing to make a Subject Access Request under Article 15 of the [EU GDPR / UK GDPR] and the Data Protection Act 2018 for personal data concerning my child, [child's full name], date of birth [DOB], a patient at [practice / hospital].

I am the child's [father / mother] and hold parental responsibility. A copy of [the birth certificate / court order] is enclosed.

Please provide a copy of the complete medical record, including:

  1. All consultation notes and clinician's narrative entries.
  2. All appointment-attendance records, including: date scheduled, date attended (or DNA), and who brought the child.
  3. All referrals made and received.
  4. All correspondence sent or received about my child (including from social care, school, or other healthcare providers).
  5. Any safeguarding flags or notes.
  6. Any records of my child's expressed views regarding family contact, if recorded.
  7. Information about any third parties to whom records have been disclosed.

I understand that under Article 15, the response is due within one calendar month, without charge.

After you receive the response

  • Cross-reference appointment-attendance records to your contact log — gaps where the other parent claimed the child was with them but the medical record shows missed appointments are dispositive.
  • Note any clinician entries recording the child's stated views — these are admissible third-party records.
  • Save the complete pack for any future expert engagement.
